Being referred to a program does not mean someone knows how to enter it. Clarify requirements, documentation, decision ownership, and the handoffs between programs so participants understand their next step and staff can provide accountable support.

Participant Goals & Pathway Planning

A list of program activities does not always give someone a workable route forward. Connect the participant’s goals to meaningful steps, practical support, responsibilities, and review points so the plan can guide action and adapt over time.

Prior Experience & Capability Recognition

People often bring more experience than their records make visible. Examine how relevant activities, responsibilities, and evidence can support a clearer account of capability—and identify what requires further review before it informs a pathway decision.
